Call number 2 is put on hold, as expected, and you are given the options to merge the two calls, or to flip the two calls and put the first call on hold.
Expected behavior
Both options (flip and merge) will work, allowing you to merge the two calls.
Actual behavior
Neither work, you are stuck with call 1 and are not able to respond to call 2.

My issue. There is nothing, button or window, that allows to switch back. I just have my home screen showing. I pull up the phone app again then hange up and then call the holding number back.
I too have this issue on my Android 12, Moto phone. One thing I know is related - and may affect the original poster as well - is that I have the option enabled to always display incoming calls full-screen. If I turn that off, I do indeed get a notification and can use it to at least switch to the 2nd call.
It does seem that the 2nd call is displayed and is displayed full-screen. However, there is no way for me to pick up that 2nd call. Nothing on the screen seems active other than the red, hang-up button.
