Entity statements for issuers should be stored as a block

Broken out by header, ES, and signature. Header and signature should be stored separately for the fetch statements (coming from the trust registry).

This is because the validity of the fetch statement should also expire at the time when the issuer-provided ES expires (i.e. the internal contents of the ES should be identical)
