Milestones app issue in MS Teams #99

Milestones app is allowed in our Teams admin center. However, facing two issues -

we are not able to add this app in our Teams channel
Teams members who were already using Milestones app is facing error message " Sorry there's been a disconnect" (screenshot provided)

Any idea what could be the issue here
