Add name of form that note was submitted from to the notes panel.

It's useful to have context about which form a note was added from, so we should add the name of the form to the meta-info for each note in the notes panel.

A terrible terrible mock:

This is fiddly because the Android client doesn't sync encounters directly, and in OpenMRS, the form info is attached to the encounter. We'd either have to add this field to the observations downloaded from the server (short term / hack fix), or restructure the client to sync encounter info as well (better, long term fix).
