should we remove Appveyor integrated builds? GitHub Actions seems to be a little more reliable and quicker

Remove AppVeyor integration

This addresses #283.
AppVeyor is less convenient than Github Actions as it is much slower to
complete multiple builds. This also has the added cost of needing to
maintain two separate build script formats rather than a single one.
