[Minor Fix] Remove duplicate lines in CHANGELOG.md for Version 4.0 #2962

There were several duplicate lines in the V4.0 Changelog, so I removed them
replace the old one with the fixed version

My guess is this was caused by a merge issue of some kind. |

I am the lone sole soul that reads the changelog and noticed the duplicate lines, if there was a reason for it then please inform me but I figured it wasn't intentional
